• Proviso Township High Schools D209 offers reduced or waived fees for families that meet certain income criteria. We follow the free and reduced lunch household income tiers to determine your eligibility. You can view the 2023-2024 income guidelines HERE.

    During the registration process, you may need to provide information for all household members, including children. Additional information may be needed to determine eligibility. 

    You may be asked to provide any of the following documents to support your application:

    • Two of the most recent paycheck statements (cannot be older than 30 days)
    • Current year tax return forms (Forms 1040, 1040A, 1040EZ, 1040NR, 1040NR-EZ)
    • Current year Social Security award letter(s) or pension statement
    • Supplemental Nutrition Assistance Program (SNAP) approval letter (NOT the EBT card)
    • TANF Letter of benefits
    • Unemployment insurance benefits certification letter
    • Notarized disability letter from employer
    • Notarized letter from employer, if any family member's wages are paid in cash
    • Court-approved child support Letter (call Child Support Division of Cook County at 312-345-4125 or 312-345-4015 for documentation)
    • Section 8 housing certificate
    • Evidence of a significant loss of income due to severe illness or injury in the family
    • Evidence of unusual expenses incurred as a result of fire, flood, storm damage, or other emergency situations

    Your information may be reviewed by a social worker to determine eligibility for a registration fee reduction or waiver.

    For fee-waiver purposes, “Student Fees” do not include the following: student ID card replacements; library fines, and other charges incurred for the loss, misuse or destruction of school property (e.g. books, musical instruments, athletic or laboratory equipment); charges for optional travel undertaken by a school club or group of students outside of school hours (e.g., senior class trip); charges for admission to school dances, athletic events or other social events; optional community service programs for which fees are charged (recreation programs, summer school).
