Proof of Residency Requirements
The following are the required documents to prove residency for enrollment in Proviso Township High Schools D209. You must fulfill both Category A and Category B.

| Real Estate Tax Bill | Gas, Water, Electric Bills |
| Mortgage Document/Payment Book | Driver's License or State ID |
| Signed Lease | Phone Bill (not cell/mobile) |
| Section 8 Letter | Cable Bill |
| Military Housing Letter | Medical Card with current address |
| Residency Attestation with lease-holder's | Vehicle Registration |
| signed lease or mortgage papers (see link below) | City Sticker Receipt |
| Bank Statement | |
| Check Stub |
Please contact your school's registration staff if you are having trouble collecting all three documents. The numbers are below:

The district may require a home visit and/or additional documentation to verify residency.
If your address is a temporary living arrangement, you will be able to enter McKinney-Vento Program information during the online registration process.
