Proviso Township High Schools District 209 will welcome back all students for the 2015-16 school year on August 17.

However, before reporting back to school, all students in the district must have at least a ninth-grade Illinois physical and up-to-date immunizations on file.

However, before reporting back to school, all students in the district must have at least a ninth-grade Illinois physical and up-to-date immunizations on file, including the Tdap vaccination for ninth-graders and meningococcal conjugate vaccination (MCV) for seniors.

In addition, incoming ninth-graders are required to provide proof of Tdap vaccinations along with the required school physical forms. Those sophomores, juniors, and seniors who have not turned in proof of Tdap vaccinations also must provide the proof. The only exceptions to this rule are an approved medical or religious exemption on file with the school or an appointment to receive the Tdap shot during the school year.

Also, incoming seniors will be required to show proof of recent meningococcal conjugate vaccination (MCV).  All 12th-graders will need to show proof of receiving two dosesunless the first dose was administered after 16 years of age. In this case, only one dose after 16 years of age is required. This vaccination will prevent your student from a severe form of bacterial meningitis as well as other illnesses caused by this bacterium, according to the Illinois Department of Public Health. A letter to parents from the Illinois Department of Public Health in English and Spanish is available for viewing below.

"These rules are necessary to ensure that we as a district comply with state law and Illinois School Code," said Dr. Nettie Collins-Hart, superintendent of District 209. "Above all, however, we need to ensure the health and safety of all of our students. Parents need to make sure that their students have these necessary medical forms on file to ensure the students can begin the new school year on a good note and to avoid any disruptions in the school year."

Additional information and some best practices on immunizations are available on the Illinois State Board of Education website at www.isbe.net/research/pdfs/immunization-guidance.pdf.

Students at Proviso East High School and Proviso Mathematics and Science Academy may contact the Proviso East School Based Health Center for physicals at (708) 449-9522. Students at Proviso West High School and those needing immunizations may contact available clinics within the area.
