Forest Park, IL...Proviso Township High Schools District 209 will welcome back all students for the 2013-14 school year on August 19.
However, before reporting back to school, all students in the district must have at least a ninth-grade Illinois physical and up-to-date immunizations on file.
Under its First Day Exclusion Policy, no student will be admitted on the first day of school unless their ninth-grade Illinois physical and current immunizations are on file at their respective school. Parents need to ensure that their students’ physicals are complete and include details of the students’ health history. Parents also must answer all of the questions on the physical form, sign and date the form, and complete the diabetes screening questions that are attached.
In addition, incoming ninth-graders are required to provide proof of Tdap vaccinations along with the required school physical forms. Those sophomores, juniors, and seniors who have not turned in proof of Tdap vaccinations also must provide the proof. The only exceptions to this rule are an approved medical or religious exemption on file with the school or an appointment to receive the Tdap shot during the school year.
"These rules are necessary to ensure that we as a district comply with state law and Illinois School Code," said Dr. Nettie Collins-Hart, superintendent of District 209. "Above all, however, we need to ensure the health and safety of all of our students. Parents need to make sure that their students have these necessary medical forms on file to ensure the students can begin the new school year on a good note and to avoid any disruptions in the school year."
Students at Proviso East High School and Proviso Math and Science Academy may contact the Proviso East School Based Health Center for physicals at (708) 449-9522. Students at Proviso West High School and those needing immunizations may contact available clinics within the area.
